Key Responsibilities:

  • Perform comprehensive physical assessments and analyze medical histories to evaluate patients’ conditions accurately.
  • Create and implement tailored treatment plans that address individual rehabilitation goals and patient needs.
  • Conduct therapeutic exercises and training programs to improve mobility and enhance overall functionality.
  • Educate patients and their families about injury prevention, wellness strategies, and the recovery journey.
  • Monitor and document patients’ progress, adjusting treatment plans as necessary to achieve rehabilitation goals.
  • Collaborate with a multidisciplinary healthcare team to ensure holistic patient care.
  • Maintain meticulous patient records and documentation, complying with regulatory standards.

If you bring the following, we’d love to connect:

  • Currently licensed to practice as a Physical Therapist in the state of agency operation, in full compliance with accepted professional standards and practices
  • Minimum of nine months of hands-on clinical experience as a Physical Therapist
  • Community or home health experience strongly preferred

Required Certifications & Licensures

  • Master’s degree in Physical Therapy from a CAPTE-accredited program
  • Current CPR certification, maintained continuously throughout employment in a clinical role
  • Valid driver’s license with the ability to travel across all business locations as needed
